BP: Gulf Oil Spill Caused by a "Sequence of Failures"

BP released a report yesterday indicating that the explosion on the Deepwater Horizon oil rig was the result of "complex and interlinked series of mechanical failures, human judgments, engineering design, operational implementation and team interfaces." The April 20 accident killed 11 people and released millions of gallons of oil into the Gulf of Mexico.

The report goes on to say that services provided by Transocean and Halliburton may have contributed to the accident. Responding on behalf of Transocean, representative Lou Colasuonno described the findings as "a self-serving report that attempts to conceal the critical factor that set the stage for the Macondo incident: BP’s fatally flawed well design."
